NDIS Support Coordination is a service offered under the National Disability Insurance Scheme (NDIS) in Australia. It assists in coordinating and access of support services, to help people with disabilities and their carers and meet their individual needs. These services provided by NDIS Support Coordination include identifying and connecting with service providers, coordinating assessments, planning for transitions, and monitoring plan outcomes and expenditures. Support Coordination’s goal is to build the capacity of the individual with a disability and to help manage their support and achieve their goals.
Access for NDIS Support Coordination help is dependent on your individual needs and goals. If your NDIS plan is approved, you may be eligible to receive funding for support coordination to help you reach your goals. The availability of this funded support in your plan is determined by factors such as your specific disability or circumstances.
Getting NDIS Support Coordination assistance is easy. To start, submit a request for service that outlines your goals and support needs. Once received, a support coordinator will reach out to you to discuss your specific requirements. They will then evaluate your needs and offer advice and guidance on how to optimally utilize the supports in your plan. In addition, specialist support coordinators can also help overcome any obstacles to ensure that you can access the right supports.
To be eligible for Support Coordination through the National Disability Insurance Scheme (NDIS), you must meet certain criteria. These criteria include having a limited support network, being new to the NDIS, or having the potential to greatly benefit from the NDIS. Support Coordination is a capacity-building support that helps you make the most of your plan's supports. It assists you in finding the best services to meet your needs and goals within the NDIS. Once you receive your NDIS plan, a support coordinator can help you by reviewing it and determining what supports you need to fully benefit from your plan. Please note that not all NDIS participants are eligible for Support Coordination, and it is only funded for those who are likely to greatly benefit from it.
There are three levels of support coordination that can be funded by the NDIS in your plan:
We've been working with Mark for over two years now. Mark has some conditions that are eligible for NDIS, and we've been supporting him with his plan. We offer services like assistance with daily living and short-term accommodation, and Mark has been super happy with the care he's received from us. It's been great to see his quality of life improve, and we're honoured to be a part of that journey with him.
We're always here to answer any and all of your questions. Below are some of the more common questions we receive, however if you don't see your question amongst these, please don't hesitate to give us a call or drop us a line.
The responsibilities of an NDIS support coordinator are to be your advocate and guide throughout your NDIS journey. Whether it's helping you to understand your NDIS plan, maximizing your funded supports, or connecting you with community and mainstream services, your support coordinator is here to support you every step of the way. They can also assist with building your independence, strengthening connections with loved ones and carers, and connecting you with trusted providers. So don't hesitate to reach out to them for any help you may need!
NDIS support coordination helps people with disabilities to gain access to the resources, services, and supports they need to live an independent life. It helps in setting goals and finding the right services to achieve them, providing guidance on using NDIS systems and processes such as finding housing and educating advocates. The support coordinator ensures the best possible experience with the NDIS and helps access the necessary services to reach desired goals, while also ensuring the NDIS plan is implemented. Support coordinators also help to ensure that the NDIS plan is implemented effectively and the participants are getting the maximum benefit from the funding.
Eligibility for NDIS funding for support coordination is dependent on your individual needs and goals. Once your NDIS plan is approved, you may receive funded support coordination to assist you in achieving your goals. Getting this type of funded support in your plan depends on your individual needs and circumstances, such as your disability or your situation.
You can look up registered support coordination providers located near you using the Provider Finder tool in the NDIS myplace portal, or online. Additionally, do not hesitate to reach out to your Local Area Coordinator, Early childhood partner, family, or friends to assist you in locating support coordinators in your area. Make sure to consider your support coordinator and ensure they are the right person for you. It is recommended that a service agreement be set in place and your support coordinator can help you in understanding it further.
Your support coordinator should always make sure you have choice and control over your goals and support. They must treat you with respect and communicate with you in your preferred communication style. Support coordinators must develop your plan in your best interest and keep up-to-date with your current NDIS processes and rules set out by the NDIS commission, so you don’t need to worry about the little things.
Support coordinators achieve this by breaking your choices down into smaller parts, or providing you with different options, so you can understand your decisions clearly. Your support coordinator guides you every step of the way in your NDIS journey. They help you make sense of your plan, get the most out of your funding, and find the resources and services you need in the community. They'll support you in becoming more independent, strengthening your relationships with those you care about, and connecting you with providers you can trust.
NDIS support coordinators offer direct support services like managing your NDIS plan, giving you expert advice, introducing you to great service providers, handling paperwork, and anything else you need help with. Your coordinator will work closely with other professionals to make sure you get the best possible outcomes. And if there's anything you need that isn't covered by the NDIS, your coordinator will help you find it and make sure you have the support you need. From respite care to individual support, they've got you covered.
If you'd like a personal advocate and guide to help you navigate your NDIS journey, then we highly recommend you seek support coordination. A support coordinator will assist you in any way they can, based on your unique preferences and choices. They will help you comprehend your NDIS plan, get the most out of your funded support, and connect you with community and mainstream services. They're here to help build your independence, strengthen your connections with loved ones and carers, and find trusted providers. They'll also ensure that all your service agreements and bookings are in order. So, if you're ready for some extra support, don't hesitate to reach out to an NDIS support coordinator!
The NDIS incurs costs for support coordination services, though the specific costs depend on the level of support required.

Level 3 Support Coordination (Specialist Coordination) employs an expert or specialist approach to those who have specific high complex needs or high-level risks in a participant’s situation. This is delivered by appropriately qualified and experienced practitioners such as Psychologists, Occupation Therapists, Social Workers, or Mental Health Nurses.
The role of Specialist Support Coordination is to help NDIS participants overcome barriers to accessing the right support and implementing their plan. They assist in reducing complexity and connecting with both funded and broader support systems. They work to negotiate appropriate support solutions with multiple stakeholders and design a comprehensive service plan to address barriers and promote plan implementation. They can also design a complex service plan that encompasses how all the stakeholders in a participant’s life will interact to resolve barriers and promote appropriate plan implementation. Once developed, a Specialist Support Coordinator will continue to monitor the plan, but it may be maintained by one of the participant’s support workers or other care supports. Additionally, when providing services to clients with disabilities under the NDIS, support coordinators are only allowed to claim travel costs directly related to providing support or service to participants, including non-labor costs such as road tolls, parking, and public transport fares.
